information be completed prior to your visit. This information provides our audiologists with your
Hearing History and Patient Information so they are totally prepared for the time they spend with you.
In this packet of information, we have included a map and directions to our office for your convenience. Please

Are you coming in for a VNG Test? Are you coming in for a VNG Test for Balance and Dizziness?
If you have scheduled an appointment with our Balance & Dizziness Clinic
to have your balance or dizziness evaluated,
you will be meeting with Chris Cummins-Beagle or Kim Gates, who will work
with you at our Civic Center Office, located in downtown Scottsdale.
Our comprehensive evaluation will provide you and your physician with important
information about your balance system. We will make recommendations based on your
medical condition, lifestyle and other individual needs.
In order to make this appointment as comprehensive as possible, we require that certain
